Terms & Condition of Membership
1. All members must complete a current Revolution Martial Arts Academy application form before being allow to participate in lessons.
2. Annual (non-refundable) Insurance fee including licence and membership is payable on the students second lesson.
3. Revolution Martial Arts Academy will upon receipt of the student’s annual fees, issue a licence, insurance certificate and grading book.
4. Training fees are payable in monthly advanced .
5. Training fees are calculated over 12 months (Jan-Dec) which includes occasional club closure (including but not limited to Bank Holidays)
6. Non-payment of training fees will automatically cancel the student’s membership.
7. In the interest of health and safety all students must wear a gum shield if student forgets theirs they can be purchased via club for £2.50.
8. Always inform the instructor of any injuries you may have or if you feel unwell.
9. For safety, finger and toe nails should be kept short.
10. If you lose you graining book your will be unable to grade, a replacement grading book will cost £10.00
11. Children must not recreate anything learned in the gym outside the gym.
Miscellaneous
· In case of a fire, please follow the instructor’s directions.
· In the event that there is a problem with the current training location, classes may move to alternative venue temporarily. We will endeavour to give as much notice as possible of any changes but we cannot be held responsible for amendments are beyond our control.
· Any unacceptable behaviour will not be tolerated and may result in those people being asked to leave.
· Revolution Martial Arts Academy reserve the right to cancel memberships at any time and for any reason, and to refuse new applications for membership.
· Please feel free to speak to any of the instructors about any concerns, comments or complaints and we will do our best to resolve any issues. It is extremely important to us that you are happy with your training and membership, and we welcome your feedback on how things are going.
Declaration Of Informed Consent & Assumption Of Risk
Taking part in any style of martial art or boxing carries inherit risks and dangers. As a club we do everything we can to ensure you are safe & protected during training sessions – but accidents can and do happen.
By taking part in our club, you agree to assume all risks involved in the practice of martial arts, boxing & self defence as per the classes requirements and details.
Some of the common risks involved are as follows;
· Trips, Slips & Falls
· Contact Injuries
· Combat Injuries
· Back, Neck And Head Injury
· Bruising & Contusions
Of course, this is not an exhaustive list of injuries that can occur during training within our club. By choosing to join our club & train with us you are agreeing to assume all risks involved and understand that by giving your informed consent, you are releasing our club, it’s instructors, members and other associates from any and all other positions of liability or responsibility.
This means that yourelease, discharge, and forever hold harmless the instructor, attached Association, club, members, officers, employees or any other associated individuals as you assume all risks involved with training.
Please risk assess any venue or competitive situation to ensure your safe & protected. It’s up to you to ensure all equipment you use is protective & up to the required standards.
This form hereby releases us from any liability and assumes all risk entirely to yourself.
